Published in the Aesthetic Surgery Journal, this peer-reviewed prospective research is the most detailed measurement-based comparison of implant stability to date.<\/p>\n<h3 class=\"text-text-100 mt-3 -mb-1 text-&#091;1.125rem&#093; font-bold\" data-sourcepos=\"7:1-7:26;384-409\"><strong>Why This Study Matters<\/strong><\/h3>\n<p class=\"font-claude-response-body break-words whitespace-normal\">Surgeons choosing a breast implant weigh many factors, including shape, volume, gel firmness, and surface texture. However, long-term objective data on how those choices actually play out in real patients has been limited. Much of the available evidence relies on subjective assessment or short follow-up periods, leaving surgeons without a reliable picture of how implants behave over time.<\/p>\n<p class=\"font-claude-response-body break-words whitespace-normal\">Dr. Giovanni Bistoni, Dr. Pietro Luciano Serra, and Dr. Melba Lattanzi set out to change that. Their prospective study, published in the Aesthetic Surgery Journal (Volume 46, Issue 7, 2026), placed Arbrea at the center of the methodology.<\/p>\n<hr class=\"border-border-200 border-t-0.5 my-3 mx-1.5\" \/>\n<h3 class=\"text-text-100 mt-3 -mb-1 text-&#091;1.125rem&#093; font-bold\" data-sourcepos=\"15:1-15:77;1054-1130\"><strong>Study Overview: 106 Patients, 5 Implant Types, 12 Months of 3D Monitoring<\/strong><\/h3>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">T<\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"1\">he research team enrolled 106 patients <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"40\">(212 breasts) undergoing primary <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"73\">breast augmentation at a <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"98\">single private practice. <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"123\">Recruitment took place over two <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"155\">years, from June 2022 to June <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"185\">2024. Each patient was then <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"213\">followed individually for 12 <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"242\">months postoperatively. All <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"270\">procedures were performed by the <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"303\">same surgeon using a <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"324\">dual-plane technique through the under the breast<\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"357\">\u00a0fold incision. This <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"390\">deliberate design choice minimized <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"425\">variability and isolated implant <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"458\">characteristics as the primary <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"489\">variable.<\/span><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">Five implant types were <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"24\">included in the study:<\/span><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<ul class=\"&#091;li_&amp;&#093;:mb-0 &#091;li_&amp;&#093;:mt-1 &#091;li_&amp;&#093;:gap-1 &#091;&amp;:not(:last-child)_ul&#093;:pb-1 &#091;&amp;:not(:last-child)_ol&#093;:pb-1 list-disc flex flex-col gap-1 pl-8 mb-3\">\n<li class=\"font-claude-response-body whitespace-normal break-words pl-2\">Progressive Gel ULTIMA (PgU), smooth round<\/li>\n<li class=\"font-claude-response-body whitespace-normal break-words pl-2\">HSC+ gel, smooth round<\/li>\n<li class=\"font-claude-response-body whitespace-normal break-words pl-2\">Round, microtextured (HSC+ gel)<\/li>\n<li class=\"font-claude-response-body whitespace-normal break-words pl-2\">Anatomical, microtextured (HSC+ gel)<\/li>\n<li class=\"font-claude-response-body whitespace-normal break-words pl-2\">Anatomical, polyurethane (PU) coated<\/li>\n<\/ul>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">Surgeons assessed <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"18\">patients preoperatively, at 6 <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"48\">months, and again at 12 months. <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"80\">At each visit, the team <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"104\">used Arbrea software to <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"128\">perform 3D measurement-based <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"152\">analysis from standardized 2D <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"182\">photographs. Two key indicators <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"214\">of implant behavior were <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"239\">measured at every follow-up:<\/span><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<ul class=\"&#091;li_&amp;&#093;:mb-0 &#091;li_&amp;&#093;:mt-1 &#091;li_&amp;&#093;:gap-1 &#091;&amp;:not(:last-child)_ul&#093;:pb-1 &#091;&amp;:not(:last-child)_ol&#093;:pb-1 list-disc flex flex-col gap-1 pl-8 mb-3\">\n<li class=\"font-claude-response-body whitespace-normal break-words pl-2\"><strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"4\">Nipple-to-under the breast\u00a0fold distance (N-IMF):<\/span><\/strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"4\"> a direct measure of <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"72\">lower pole stretching, recorded <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"104\">in centimeters<\/span><\/li>\n<li class=\"font-claude-response-body whitespace-normal break-words pl-2\"><strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"123\">Lower pole ratio (LPR):<\/span><\/strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"123\"> el <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"153\">percentage of lower pole volume <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"185\">relative to total breast volume<\/span><\/li>\n<\/ul>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<hr class=\"border-border-200 border-t-0.5 my-3 mx-1.5\" \/>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<h3 class=\"font-claude-response-body break-words whitespace-normal\"><strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"2\">Why These Measurements Matter<\/span><\/strong><\/h3>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">Lower<\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"5\"> pole stretching is one of the most <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"41\">clinically significant changes <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"72\">that can occur after breast <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"100\">augmentation. Too little stretching <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"136\">in the early <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"149\">postoperative period can indicate a <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"185\">tight and unnatural result. <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"213\">On the other hand, too much stretching<\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"241\">, particularly in patients <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"278\">with tissue looseness, can lead to <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"310\">bottoming out, implant <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"333\">displacement, and revision surgery. <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"369\">Tracking N-IMF and LPR over time <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"402\">therefore gives surgeons an <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"430\">objective view of how the <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"456\">implant is interacting with <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"484\">surrounding tissue. Before tools <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"517\">like Arbrea, surgeons had to <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"546\">rely on visual assessment alone.<\/span><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<hr class=\"border-border-200 border-t-0.5 my-3 mx-1.5\" \/>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<h3 class=\"font-claude-response-body break-words whitespace-normal\"><strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"2\">Key Findings: Not All Implants Behave the Same<\/span><\/strong><\/h3>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">The study found a <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"18\">clear ranking of implant stability <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"53\">across all five groups. <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"77\">Overall, surface texture and gel firmness<\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"110\">\u00a0were the two factors that <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"147\">made the biggest difference.<\/span><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"2\">Smooth Implants: The Most Change Over Time<\/span><\/strong><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">First, smooth PgU round <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"24\">implants changed the most. Between 6 <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"61\">and 12 months, the <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"80\">nipple-to-under the breast\u00a0fold distance <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"117\">increased by nearly 1 cm on average, <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"154\">and the lower pole volume share grew by <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"194\">almost 7 percentage points. This <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"227\">happens because the softer PgU gel <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"262\">shifts downward when the patient is <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"298\">upright, gradually stretching the lower <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"338\">pole over time. As a result, the <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"371\">outcome can look natural and full. <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"406\">However, it also means these implants <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"444\">carry more risk in patients with soft <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"482\">or loose tissue. Similarly, smooth HSC+ <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"522\">round implants showed moderate stretching<\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"553\">, roughly half that of PgU <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"590\">implants, with better upper pole <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"623\">fullness preserved. Nevertheless, <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"657\">measurable change still occurred <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"690\">between 6 and 12 months.<\/span><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"2\">Textured Implants: Better Control, More Predictability<\/span><\/strong><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">In contrast, <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"13\">microtextured implants, both round and <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"52\">anatomical, sat comfortably in the <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"87\">middle. Adding surface texture and <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"122\">increasing gel firmness slowed down <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"160\">tissue adaptation without stopping it <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"198\">entirely. Therefore, these implants <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"234\">offer a strong middle ground for <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"267\">patients who need positional control <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"304\">but still want a natural feel.<\/span><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><strong><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"2\">Polyurethane-Coated Implants: Virtually No Movement<\/span><\/strong><\/p>\n<\/div>\n<div class=\"_chunkWrapper_6ta1u_30\">\n<p class=\"font-claude-response-body break-words whitespace-normal\"><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"0\">Most remarkably, <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"17\">polyurethane-coated anatomical implants <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"57\">barely moved at all. Over the full <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"92\">12-month period, the average change in <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"131\">lower pole distance was just 0.04 cm, <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"169\">which is statistically zero. This is <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"206\">because the polyurethane coating <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"239\">encourages surrounding tissue to grow <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"277\">into the implant surface early on, <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"312\">effectively locking it in place. <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"345\">Consequently, PU-coated implants proved <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"385\">to be the most stable option, <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"415\">particularly for active patients or <\/span><span class=\"_animating_6ta1u_10\" data-newtext-seq=\"451\">those with tissue looseness.<\/span><\/p>\n<hr class=\"border-border-200 border-t-0.5 my-3 mx-1.5\" \/>\n<\/div>\n<h3 class=\"font-claude-response-body break-words whitespace-normal\"><strong>Complications: Less Stability Means Higher Risk<\/strong><\/h3>\n<p class=\"font-claude-response-body break-words whitespace-normal\">The overall complication rate across the entire cohort was 2.4% per implant. Importantly, all complications occurred in the smooth round groups. Furthermore, smooth PgU round implants showed a statistically significant association with repeat surgery (p = .011). Complications included one case of medial implant displacement linked to rib cage asymmetry, and three cases of inferior displacement associated with large implant volumes and failure to follow postoperative activity restrictions. Notably, no capsular contractures, infections, hematomas, or ruptures were recorded across any group. In contrast, the PU-coated, microtextured round, and microtextured anatomical groups recorded zero complications, supporting the idea that surface fixation mechanisms reduce early dislocation risk.<\/p>\n<hr class=\"border-border-200 border-t-0.5 my-3 mx-1.5\" \/>\n<h3 class=\"font-claude-response-body break-words whitespace-normal\"><strong>Patient Satisfaction: High Across All Groups<\/strong><\/h3>\n<p class=\"font-claude-response-body break-words whitespace-normal\">The team assessed patient satisfaction at 12 months using a Likert scale from 1 to 5. All groups returned high scores, and no statistically significant difference between implant types was found (p &gt; .05). In other words, patients reported satisfaction regardless of which implant they received.
